Caring Connections Alliance offers a comprehensive suite of Community Health Worker (CHW)-led services that directly address these challenges by extending the clinical workforce into the community, improving patient access, stabilizing care engagement, and reducing downstream costs. All services are delivered by trained, culturally competent CHWs, many of whom are licensed notaries and experienced in working with digitally disconnected, medically complex, and underserved populations across Florida’s rural and urban settings. Our six core service lines include:
Community-Based Intake Management
Streamlines patient onboarding for elderly, disabled, and low-tech populations by completing intake documentation in the home or field—reducing appointment lag time, improving show rates, and supporting accurate clinical risk documentation.
Referral Management & Specialty Navigation
Ensures successful connection to specialty care after referral, particularly for high-risk or chronically ill patients. CHWs eliminate common breakdowns in the referral process that lead to delayed diagnoses, disease progression, and avoidable ED use.
Social Needs Assessment & Community Referral
Conducts validated SDOH screenings and generates detailed social needs notes to inform care plans, support risk adjustment, and enable early intervention. CHWs also provide direct referrals to trusted local resources tailored to the patient’s needs.
Social Security Disability (SSD) Application Assistance
Guides eligible patients through the full SSD application process—helping them secure income and coverage while reducing care fragmentation, treatment non-adherence, and preventable system strain due to untreated functional impairment.
Medicaid Application & Redetermination Support
In response to new federal and state-level Medicaid recertification and work requirement policies, CHWs ensure eligible patients apply, reapply, and maintain coverage—safeguarding continuity of care and protecting providers from uncompensated revenue loss.
Medical Power of Attorney Facilitation (Florida-Compliant)
Enables patients to complete and notarize Health Care Surrogate Designation forms under Florida law, reducing legal ambiguity during emergencies, enhancing care planning documentation, and supporting billing for advance care planning.
