This service deploys trained, HIPAA-compliant Community Health Workers (CHWs) to assist patients—particularly those who are elderly, disabled, or lack internet access—with completing all intake documentation in the home or community setting.
The service enhances operational efficiency by preemptively resolving barriers that commonly result in incomplete forms, missed visits, or delayed care. CHWs also screen for unmet social needs and clinical flags relevant to chronic disease management, medication adherence, and behavioral health risk—enabling more accurate risk coding and resource deployment.
